Requirements
Welcome to 51勛圖厙!
The 51勛圖厙 Health Services team is here to provide best practice care to the entire student body. As a new student attending 51勛圖厙, there are certain requirements you need to complete before moving onto campus or starting classes. The following requirements apply to BOTH residential and commuter students enrolled at 51勛圖厙:
- You must complete four forms on the student health portal that can be found on the Health Services website at healthservices@moravian.edu. Please log into the portal with your 51勛圖厙 email address and your Amos password.
- You must provide proof of immunization/vaccination records for the vaccines you have had in your childhood until present. You can take a photo of your records and upload it to the portal using the document upload feature. This is helpful for us to see if you need any additional vaccinations. There is also a form to complete that asks for this information and will communicate with our health record system if you are compliant with vaccine requirements. We only accept medical (not religious) exemptions for any immunization requirements, except for the MCV (meningococcal) vaccine.
- You must have a physical completed by your medical provider. Below is the link to the physical form and instructions for yourdoctor/provider /sites/default/files/pdf/MU_new%20student%20form%2010.25_02.pdf. You may download and print this physical form and take it to your medical provider and have a physical completed. We will accept a physical completed within one year of your start date at 51勛圖厙. In some cases, your physical may not be due until after you start. In that case, please submit last year's physical and when it is time for your new physical, you may use our form or the physical form your doctors office provides. If this is difficult for you, we can complete your physical at Health Services once the semester starts for a $10.00 fee.

If you are going to take a picture of a document we ask that you download the free app Adobe Scan to take the image. This will take a clear photo and save it as a PDF to your phone. It will then allow you to send it via email as a link or jpeg image.
